Protecting your child’s privacy and the confidentiality of your child’s personal information is important to SU as it is fundamental to the way we conduct Scripture Union Victoria. A copy of the SU Privacy Statement or detailed policy can be obtained from the SU Victoria website or office. From time to time, photos taken of SU activities may be used for promoting the many benefits to participants. Information and photos from various activities may appear on the SU web site or publications. Under no circumstances will the subjects in the photographs be personally identified without prior consent.

The information in this sheet will help us plan with care and safety. It in no way prejudices your application, but must be completed and returned before we can assume care of your child.

In attending the program, you consent to your child's participation in a range of general sporting and recreational activities.

I am happy for photographs to be taken of my child during their participation in this Scripture Union Victoria camp and I am willing for Scripture Union Victoria to use these photographs.

I understand that photos taken may be shared by the team with the campers and be used by Scripture Union Victoria in promotional material, publications and included on material used by Scripture Union Victoria for fundraising.

This is a day program from Camp Coolamatong, not an overnight camp.

Please bring:

• A packed lunch (morning tea, lunch and afternoon tea)
• A drink bottle
• A hat
• Bathers and a towel
• Water shoes

Note: More detailed information about what to bring will be included in a confirmation email from Camp Coolamatong.

Please clearly label all of your child’s belongings!

Sunscreen and mosquito repellent will be provided by Coolamatong, but can also be packed to bring.

Drop off is between 8am and 9am with program commencing at 9am.
Pick up is between 4.30pm and 5.30pm with program finishing at 4.30pm.

All leaders have a WWCC and ChildSafe training.

• shows symptoms of COVID-19 virus
• has been tested for COVID-19 virus and is awaiting results
• is currently in quarantine due to COVID-19 pandemic stipulations.

I understand that, if my child displays symptoms of COVID-19 virus during the day program, I will be responsible for picking them up from the program immediately.

I acknowledge and I understand that certain inherent risks and dangers may exist in the activities in which my child will be participating. I acknowledge that while Scripture Union and its leaders will take reasonable steps to provide a safe environment and to ensure all activities and equipment supplied by them for such activities is of a reasonable standard, Scripture Union Victoria will not be liable for any injury that may be suffered by my child, which arises either directly or indirectly from, or in connection with, the activity described.

I hereby agree to indemnify Scripture Union against any and all claims arising from, or in connection with, any injury that may be suffered by my child, or that they may cause to another person, as well as any loss or damage to property, equipment or belonging to my child, or any other person, arising either directly or indirectly out of or in connection with the activity described.

I agree that Scripture Union may authorize on my behalf whatever medical treatment my child may require. (This includes, but is not limited to, ambulance attendance and hospital treatment). I agree to pay all medical expenses incurred.

I give consent for photos of my child to be taken while participating in this activity. I acknowledge they may appear on SU’s online platforms and publications but recognise that my child won’t be identified.

I confirm that the information contained in this application is true and correct.
